This form is ONLY to be used by the Agency and/or Supplier placing the ONE advertisement that is included with the agency's registration for the FALL 2008 Directory. Please refer to the METRO/jsix Directory Advertising Form for all ads to be placed by other suppliers. The ad will be positioned facing the agency listing in the Directory. The cost is $150.00 for a full page, inside. If the agency or supplier wish to have an ad positioned in a location other than adjacent to the agency listing, the additional charges would be as follows:
	Inside Front Cover	$200.00
	Inside Back Cover	$185.00
	Back Cover	$200.00
	Full Page, Inside	$150.00
	Additional ads by same supplier	$100.00 per page

The agency will be charged these additional amounts at the time of processing their registration. Please complete all three steps indicated below. 1. Fill out the information indicating what Agency the ad is being placed for, the name and contact number of the Supplier. 2. Fax this information sheet to Judith Shandro at the METRO/jsix office. Please notify Culley Creative by email or telephone as to how your ad will be created under the descriptions listed below. Send artwork to Culley Creative at address below or email as indicated. Be sure to specify what agency the ad is being placed for when communicating with Culley Creative. PLEASE NOTE: Due to the amount of ads being placed and coordinated, a $50.00 min charge will be applied to any ads not meeting specifications listed below. Any changes to ads submitted will also be charged a $50.00 min.
